If you do not have access to the Adobe Acrobat Reader to open the PDF forms, you can download the app for free from the Adobe website.

After opening the form in your web browser:

  • In Microsoft Edge, fill the form and click the Save icon in the ribbon at the top of the page, or press Ctrl (or Command) and S on your keyboard.
  • In Google Chrome or Firefox, fill the form and click the Download icon in the ribbon at the top of the page then select “With your changes” or press Ctrl (or Command) and S on your keyboard.
  • Signature format:

If you intend to file your forms at the courthouse, you must write your signature by hand.

If you intend to electronically file (e-file) your forms, you can electronically sign by adopting the format /s/your full name

  • What is our judicial district? We are the FOURTH or 4th judicial district. The fourth judicial district covers Ada, Boise, Elmore, Valley counties.
  • Write or type the forms in Black Ink
  • Do not print double sided. When you file with the court your paperwork must be single sided.

  • Filing Fees: If you have questions about fees, filing, or status updates about a case please call the Ada County Clerk’s Office at 208-287-6900 option #4, 8AM to 5PM, Monday to Friday.
