If a loved one has passed and you need help paying for their cremation or burial, please complete the cremation assistance application. This application can also be submitted by a non-family member as well as a funeral director. The application must be filed before the cremation or burial services take place. Please use the deceased’s personal and financial information when completing the application.
To ensure we can read your application, please type your answers directly into the form. It is important to note, once you start your application you cannot leave the page or you will lose any information you’ve entered.
Once you’ve completed your application, review it, print it and bring it to our office, 252 E. Front St., Suite 199, in downtown Boise. The application needs to be signed. Once you submit your application, the same day interview will be conducted by a deputy clerk.
Interviews are conducted on a first-come, first-served basis at our office Monday through Friday from 8:00 a.m. to 3:30 p.m.
