Do I need to have my application filled out before my appointment?
We recommend that you complete an application prior to your appointment so that your appointment time can be dedicated to processing the application.  Applications can be downloaded here. Additionally, here are some helpful tips when printing your application:

  1. Print on Single-Sided paper. We cannot accept double-sided forms.
  2. Compare your printed form to what appears on your screen.
  3. Make sure all data blocks and the barcode appear clearly and in the same format.
  4. If there are any distortions, smudges, missing blocks or data, or fading, it may be due to variations in printer types, printer drivers, or low-ink levels.
  5. If you cannot correct these problems after checking your printer, do not submit the form.

Instead, download and print a blank form and complete it by hand in black ink

Do I get my documents back after the appointment?
Your certified copies are sent with your passport application to the Department of State.  You will receive these documents back after processing, but they will come separately from your passport.

I had a court ordered name change, what do I need to provide?
If your name differs from your birth certificate due to a court order, you must include a certified copy of the court order with your application.

I had a passport as a minor, can I renew that now that I’m an adult?
No, you will need to fill out a DS-11 form, and it will be treated as a newly issued passport.

Why do I need to bring so many checks?
The Department of State recommends having one check per application to allow for quicker processing times.  For example, a family of five would want to bring six checks with them – one to include with each respective application, and one to cover all Ada County processing and photo fees.

Where can I get my photo done?
Our office is able to take your photo at the time of your appointment. The fee for photos is $15.00 for anyone 16 years and older. We do not charge for anyone under the age of 16.

Can I get my renewal done at your office?
Renewals are not processed at any acceptance facility.  The best way to get a renewal done is to download a DS-82 form and mail it with your expired passport.  Renewals can be processed within five years of the expiration date of your passport.  Please be aware that if your passport exceeds that amount of time prior to your application being processed, the application will be returned and you will need to complete a DS-11 form.

I have a child that is 16 or 17 years old, how do they apply for their passport?
At least one parent must accompany the minor for their appointment.  They will need proof of citizenship and identity.  Please see the What do I need to bring for a Passport Appointment section for additional details.

How do I request a certified copy of my birth certificate?
Find more information on getting a certified copy of your birth certificate.

I lost my passport, what do I do?
Submit a DS-64 form.

I got married and need to change my name on my passport.
If your passport was issued less than one year ago, you must submit a DS-5504 form to correct any information.  If your passport was issued more than one year ago, you must submit a renewal application.
