Based on the needs identified from screening and assessment, our staff will build individual case plans and connect families to community resources. This may include supportive services such as tutoring, mentoring and life skills, and/or clinical services such as counseling, suicide intervention and substance abuse treatment. We will take time to understand each family’s accessibility issues related to transportation, scheduling and insurance.
